Club North Girls Letter of Commitment - Do not use for Boys
2024-2025 Season

To become a member of, and to participate on a Club North team, you must first read and agree to the following commitments and acknowledgements, via our online commitment form.

NOTE that providing player and parent / guardian names in the online form on July 17 or later, acts as legal signatures pertaining to joining Club North.
Payment Schedule:
  • July 17, 2024... (or the day you fill out and submit the Commitment Letter) -- After filling out and submitting the Commitment Letter, you will be taken to our online payment system where you can complete the process.
  • Once logged in, select your team by Age/Name of Coach.
  • Once a commitment is accepted and first payment is made - the entire fees will be due until completed based on payment plan selected.
  • Remaining payments will be due on August 17, September 17, October 17, November 17, and January 17, 2025.
NOTE: We recommend using a computer to fill out this online form. BUT, if using a "cell phone" or tablet, before filling out the form, scroll down to see if you can see the Submit button, if not, turn your phone sideways. (You can also use your finger to move the content Left to Right. 

NOTE: If you are using a computer at your "work place" to fill out the form, know that some office computer security systems and firewalls might not allow you to submit the form. If you try and cannot, then you must use a computer outside of your work place.

NOTE: If, on form submission, you get a RED screen... go back and review; you have inadvertently failed to provide a response in a "required" field.

  1. Other Tryouts. I understand that once I sign this contract, I am committed to the club below and that I cannot attend any other tryouts/open gyms for the 2024-25 season.

  2. Only one (1) Letter of Commitment Permitted. A player who signs a Letter of Commitment with more than one club is subject to suspension for the entire 2024-25 season.

  3. Club Signatures Required Prior to Submission. This document must be signed and dated by the club representative or club director before submission to the player or parents (or legal guardian) for their signatures.

  4. Parent/Guardian Signature Required. My parent or legal guardian is required to sign this Letter of Commitment if I am less than 18 years of age at the time of signing.

  5. Falsification of Letter of Commitment. If I falsify any part of this Letter of Commitment, including the date, I understand that I am subject to suspension for the entire season.

  6. Nullification of Other Agreements. My signature on this Letter of Commitment supersedes any Agreements, verbal or otherwise, which would release me from the conditions stated within this document. I understand that this Letter of Commitment may be amended or changed only in writing signed by the persons identified below.

  7. Binding Agreement. I understand that I have signed this Letter of Commitment with the club and not with a particular individual. If the coach or any player(s) leaves the team, I remain bound by the provisions of this document.

  8. Commitment to the Club. I understand that I by signing this Letter of Commitment, I agree to pay the disclosed club fees when due and that I will support the club in word and deed. I certify that I have read all terms and conditions in this document. I have discussed them with the club representative named within, and I fully understand, accept and agree to be bound by them.

  9. Playing Time. I understand that membership guarantees equal access for practice and training only NOT equal playing time during competitions. You are paying for training/practice time to compete for playing time.

    * In Select Division Teams - every effort will be made to provide playing time - not equal, provided team members attend regular practices, including the last 2 practices prior to each tournament.

    * In Premier and National Teams Divisions - playing time is guided by competition, team member utilization of trained skills, team focus attitudes and consistent ball control, attending regular practices & trainings, including the last few practices prior to each tournament (except in case of injury or accidents). In general, it should be understood that team members executing better during pool play will have the opportunity for more playing time during single elimination/championship matches.

In consideration of being allowed to participate in any way in the  Club North and KC Premiere Volleyball,  LLC., athletics/sports program whether involving team or individual sports and related events and activities, the undersigned acknowledges, appreciates, and agrees that:
  1. The risk of injury and/or illness from the activities involved in the program is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce the risk, the risk of serious injury does exist;
  2. The risk to have direct or indirect contact with individuals who have been exposed to and/or diagnosed with one or more communicable diseases, including but not limited to COVID-19 or other medical conditions, diseases, or maladies, and/or any mutation or variation thereof does exist and it is impossible to eliminate the risk that I could become infected through contact with or close proximity to an individual with a communicable disease;
  3. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASES or others, and assume full responsibility for my participation; and,
  4. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual, significant hazard during my presence or participation, I will remove myself from participation and bring such hazard to the attention of the nearest official immediately; and,
  5. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS CLUB NORTH & KC PREMIERE VOLLEYBALL,LLC, its officers, officials, agents and/or employees, other participants, sponsoring agencies, directors, sponsors, advertisers, and, if applicable, owners and lessors of the premises, including Parkville Athletic Club, LLC; PRTSC, LLC; Don & Faye Alexander and M&M Beans, LLC., used to conduct the event (collectively, the "Releases"), WITH RESPECT TO ANY AND ALL INJURY, ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASES OR OTHERWISE.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

FOR PARENT/GUARDIAN OF PARTICIPANT OF MINORITY AGE: This is to certify that I, as parent/legal guardian with legal responsibility for this participant, do consent and agree to his/her release as provided above of all the Releases, and, for myself, my heirs, assigns and next of kin, I release and agree to indemnify the Releases from any and all liabilities incident to my minor child's involvement or participation in these programs as provided above, EVEN IF ARISING FROM THEIR NEGLIGENCE.

By signing up for this Club North commitment form, I, as the parent or legal guardian, agrees to the above statements. I agree that typing in my name acts as my legal signature.
